My Health Check

The ‘My Health Check’ app is designed to support you following your NHS Health Check in Kent.

Record your blood pressure, cholesterol, weight and activity results onto the app and get your heart age! Then follow the advice of your health checker and the information in the app to track your progress to a healthier lifestyle.

You can download apps that can help you to quit smoking, lose weight or get your blood pressure down. Or set goals for yourself, like being more active or cutting back on booze.

This app has been designed by Kent Community Health NHS Foundation Trust and Tinderhouse and is intended for clients in Kent who are eligible for an NHS Health Check. If you haven’t had an NHS Health Check, you can still use the app, but you may not get a correct heart age score. Merekodkan tekanan darah, kolesterol, berat badan dan aktiviti keputusan anda ke aplikasi dan mendapatkan umur jantung anda! Kemudian ikut nasihat pemeriksa kesihatan anda dan maklumat dalam aplikasi untuk mengesan kemajuan anda untuk gaya hidup yang lebih sihat.

Anda boleh memuat turun aplikasi yang boleh membantu anda untuk berhenti merokok, mengurangkan berat badan atau mendapatkan tekanan darah anda ke bawah. Atau menetapkan matlamat untuk diri sendiri, seperti yang lebih aktif atau memotong kembali pada minuman keras.

Aplikasi ini telah direka oleh Kent Community Health NHS Foundation Trust dan Tinderhouse dan dimaksudkan untuk pelanggan di Kent yang layak untuk daftar NHS Kesihatan. Jika anda belum mempunyai Daftar NHS Health, anda masih boleh menggunakan aplikasi, tetapi anda tidak boleh mendapatkan skor umur hati yang betul.
